Pitch=Vibrato: multiply the frequency by 1+output of an LFO (the 1 is needed to centre the oscilations around the tuned pitch).
Volume=Tremolo: multiply the OSC output by the output of an LFO. You may want to add 1 here too or otherwise compenstate for the depth of the LFOs output. See http://www.synthmaker.co.uk/forum/viewtopic.php?t=4219
BTW - the stock LFOs have a very low upper limit (2 Hz I think) but it's very easy to increase.
